Delta Form 1A Submission

In order to be properly excused from mandatory training events, please fill out and submit the form below. All data inputted must be IAW Army regulations and format.

Authority: Beach Battalion Policy Letter dated April 2000.
Principal Purpose: Used to track excused absences within the UC Irvine Army ROTC program.
Alternate Purpose: Track number of make up hours worked by a cadet to make up for an unexcused absence.
Note: A verbal or telephonic excuse must be followed up with a Delta Form 1A or it will not be entered as an excused absence.
